Strengthening support for bereaved caregivers

Mission

Our mission is to share knowledge, skills, and resources on grief for bereaved caregivers, for their families, friends, and the health and social service workers who support them.

Caregiver Grief Connexion is supported by the Canadian Centre for Caregiving Excellence – powered by the Azrieli Foundation.

Caregiver Grief Connexion offers free and accessible educational resources on caregiver grief and bereavement for health and social service professionals and care providers.

Communities of Practice

Communities of Practice are specially designed to bring together professionals and care providers working with caregivers in the health and social service sectors to engage in knowledge transfer, peer support learning, and skills development. 

Coaching Consultations

Our team of coaches offers remote Coaching Consultations for those who are seeking to improve caregiver bereavement support services within their communities.

Telehealth Outreach Program

In order to bridge the gap in services, the Telehealth Outreach Program is a new pilot program offering support to bereaved caregivers after the death of the person they were caring for. In this pilot phase, the program is being implemented in the Montreal area in partnership with the CIUSSS West-Central Montreal local healthcare network.
